For healthcare providers using the Bloom Care platform

By creating a Bloom Care account or using any part of the Bloom Care platform, you agree to these terms. If you do not agree, do not use the service.
1. What Bloom Care Is
Bloom Care is a provider-facing platform that allows verified healthcare providers (midwives, OB-GYNs, doulas, fertility specialists, lactation consultants, naturopaths, pelvic floor therapists, and mental health therapists) to:
Invite patients to share their Bloom health data
View patient-consented health information in real time
Monitor patient progress across 7 data scopes
Bloom Care reads data from the patient's existing Bloom account. It does not store, copy, or duplicate patient health data on separate servers.
2. Medical Disclaimer
Bloom Care is a data access and communication tool, not a medical device. It is not intended to diagnose, treat, cure, or prevent any disease or medical condition. All clinical decisions remain the sole responsibility of the licensed healthcare provider.
Bloom Care does not replace electronic health records (EHR), clinical judgment, or standard-of-care protocols. If you observe concerning data in a patient's Bloom profile, follow your profession's standard clinical protocols.
3. Eligibility & Provider Verification
To use Bloom Care, you must:
Be a licensed or credentialed healthcare provider
Provide a valid National Provider Identifier (NPI) at registration
Have your NPI verified against the NPPES (National Plan and Provider Enumeration System) public registry
Be at least 18 years of age
Bloom Care verifies NPI numbers automatically at signup. Providing a false, borrowed, or inactive NPI is a violation of these terms and may result in immediate account termination and reporting to relevant authorities.
4. Your Account
You are responsible for maintaining the confidentiality of your login credentials
You are responsible for all activity under your account
You must notify us immediately if you suspect unauthorized access
You may delete your account at any time — this revokes all patient data access immediately
5. Patient Data Access & Consent
Access to patient health data through Bloom Care is governed by the following principles:
Consent is patient-controlled
You initiate a data-sharing request by inviting a patient via email
The patient must explicitly approve your request within the Bloom app
The patient selects which data scopes to share (cycle, pregnancy, symptoms, medications, appointments, journal, vitals)
The patient can narrow the scopes you requested — they cannot be forced to share more than they choose
Access is scoped and audited
You may only view data the patient has explicitly consented to share
Every data read is logged with your NPI, the data scope accessed, and a timestamp
The patient can view this audit log at any time
Audit logs are retained for compliance purposes
Revocation is instant
The patient may revoke your access at any time with immediate effect
Upon revocation, your linkToken is invalidated and all subsequent data requests will be denied
You must delete any locally cached or printed patient data upon revocation notification
No data duplication
Patient data is read on-demand via secure API — it is not copied to your device, your servers, or any third-party system
You may not export, screenshot, print, or transfer patient data to other systems without the patient's separate written consent
Bloom Care is not a data storage service — it is a data access window
6. Provider Obligations
As a healthcare provider using Bloom Care, you agree to:
Use patient data only for the purpose of providing healthcare services to that specific patient
Comply with all applicable laws, including HIPAA (if applicable), state privacy laws, and professional licensing requirements
Not share patient data with third parties without the patient's explicit consent
Not use patient data for marketing, research (without IRB approval and patient consent), or any commercial purpose
Maintain appropriate cybersecurity practices on your devices and networks
Report any suspected data breach to Bloom Care within 24 hours of discovery
7. Subscription & Payments
Bloom Care offers the following plans:
Free: Up to 3 patients — no charge, no credit card required
Starter: Up to 10 patients — $29/month or $279/year
Professional: Up to 50 patients — $79/month or $759/year
Clinic: Unlimited patients — $199/month or $1,899/year
All paid plans include a 14-day free trial. Subscriptions auto-renew unless canceled. You may cancel at any time through your account settings.
Payments are processed by Polar.sh (Polar Software, Inc.). Refunds are subject to Polar's refund policy. Bloom Care does not store credit card numbers or payment details.
If your subscription lapses, your account reverts to the Free tier. Existing patient links remain active, but you cannot invite new patients beyond the Free limit until you resubscribe.
8. Acceptable Use
You may not:
Access patient data for purposes unrelated to their healthcare
Attempt to access data the patient has not consented to share
Use automated tools, bots, or scripts to bulk-access patient data
Reverse-engineer, decompile, or attempt to extract the source code of Bloom Care
Impersonate another provider or use someone else's NPI
Use Bloom Care to harass, stalk, or inappropriately contact patients
9. HIPAA & Compliance
Bloom Care is designed with HIPAA-aligned safeguards including scoped consent, audit logging, encryption in transit (HTTPS), and instant revocation. However:
Bloom Care does not currently hold a Business Associate Agreement (BAA) with all infrastructure vendors
Providers who are HIPAA-covered entities should evaluate whether Bloom Care meets their compliance requirements before using it with real patient data
Bloom Care is suitable for wellness data sharing and non-regulated use cases. For regulated PHI, please contact us about enterprise/BAA options

10. AI Features
Bloom Care may include AI-powered features for providers (e.g., patient data summaries). These features:
Use third-party AI providers (currently OpenRouter/Anthropic Claude)
Process patient data only when you explicitly request it
Are for informational purposes only — not clinical recommendations
May produce inaccurate or incomplete information — always verify with primary data
11. Intellectual Property
All content, design, code, and functionality of Bloom Care is owned by Bloom Health and protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from any part of the platform.
12. Limitation of Liability
Bloom Care is provided "as is" without warranties of any kind, express or implied. To the maximum extent permitted by law:
We are not liable for clinical decisions made based on data viewed through Bloom Care
We are not liable for data accuracy — patient-entered data may contain errors
We are not liable for service interruptions, data loss, or security breaches beyond our reasonable control
Our total liability shall not exceed the amount you paid for the service in the 12 months preceding the claim
13. Changes to Terms
We may update these terms from time to time. Material changes will be communicated via email or in-app notification. Continued use of Bloom Care after changes constitutes acceptance.
14. Termination
We reserve the right to suspend or terminate your account if you violate these terms, use a fraudulent NPI, or engage in conduct that harms patients or the platform. Upon termination, all patient data access is immediately revoked.
15. Governing Law
These terms are governed by the laws of the United States. Any disputes shall be resolved through good-faith negotiation before pursuing legal action.
